Welcome to the office of David L. Barnes, D.O. To make your first visit efficient and effective, we ask that you complete our Patient Registration Forms prior to your appointment. Included in these forms are your HIPAA form, consent forms, patient portal information, office policies, and detailed medical history. Please print these out, complete them, and bring them with you to your appointment.
