Prepare for Your First Visit
Welcome to the office of David L. Barnes, D.O., and Physician Assistant Kathryn Albert! To make your first visit efficient and effective, we ask that you complete our Patient Registration Forms prior to your appointment. Included in these forms are your HIPAA form, consent forms, patient portal information, office policies, and detailed medical history. Please print these out, complete them, and bring them with you to your appointment.
Payment Information
We accept all major insurances. Dr. Barnes is a Preferred Provider for Blue Cross Blue Shield and Aetna. We do not accept new Medicare or Medicaid patients.
Your copayment or coinsurance is expected on the day of your appointment. We accept cash, check, Visa, MasterCard, or Discover.
